sliding Fee Discount program

Project H.O.P.E offers a sliding fee discount program for all patients based on an individual’s income and family size.  No one will ever be denied care because of the inability to pay.

Discounts are offered depending upon household income and size.
Patients will be charged a nominal fee of $5.00 to $25.00 at each visit.

To Determine Your Eligibility:

  • Complete a Sliding Fee Discount Program application at the time of your visit.

 

  • A receptionist will speak with you during check-in to determine if you are eligible for discounted services.

 

  • Return your necessary documentation to the health center reception staff within 5 to 7 business days to complete the application process.

 

Discounts are applicable to all patients that are below the 200% Federal Poverty Guidelines. 

Note:  The Sliding Fee Discount will be applied to any service received directly at Project H.O.P.E., Inc.
