Suicide and Crisis Lifeline

988 is the nationwide 3-digit dialing code for suicide and mental health crisis prevention. 988 has replaced the 1-800 number previously used by the National Suicide Prevention Lifeline. When someone contacts 988 via call, text or chat, all phones in the U.S. (mobile and landline) will connect with the crisis and suicide prevention lifeline. This will allow anyone in the U.S. to contact 988 at any time and get help designed specifically for someone dealing with a mental health or suicidal crisis. 988 support is a free service available 24/7.

Frequently Asked Questions

What should I expect at my appointment?

Patients are required to complete the following steps at each of their visits:

  • Intake with the receptionist
  • Review with the Social Worker
  • Assessment by Medical Provider
  • Insurance and Financial Counseling
What documentation do I need?

Private insurance or Medicaid is accepted, but not required.

The State of New Jersey requires that you bring to your visit the following information:

  • Two forms of ID-(one a photo)
  • Health Insurance Information
  • Families First card (if applicable)
  • Co-payment (if applicable)

What if I don't have insurance?

If you do not have insurance, you must bring with you:

  • Two forms of ID
  • Proof of Income
  • Proof of Residence
  • Documentation that you are not eligible for insurance
  • Co-payment (if applicable)

How much will my visit cost?

Project H.O.P.E. provides services to the uninsured and to others on a sliding-fee scale, based on an individual’s or family’s size and income. Project H.O.P.E. will see all patients regardless of their ability to pay. 

How long do prescription renewals take?

Prescription renewals take 72 hours to process. Project H.O.P.E. does not prescribe narcotics.

 

What if I need forms completed?

Individuals who need to have forms completed (e.g., County disability forms) must have a medical visit with the forms. Forms are available seven business days following your exam.

How do I get my medical records?

Patients may obtain a copy of their medical records by completing, and signing, and authorization of Release form that can be obtained from the medical receptionist. Project H.O.P.E. has thirty days to complete this request.

How do I transfer medical providers?

All appointments are scheduled with your routine provider.

  • To begin the transfer process, please speak with your medical provider or a member of our nursing staff.

Until the request is approved, you must continue to see your current provider.

I have a grievance.

Grievances must be submitted in writing to the Chief Executive Officer (CEO). It will be reviewed and followed-up within thirty days.
