*Minors MUST be accompanied by a parent/guardian. As proof of age, a birth certificate must be presented at the time of vaccination. Vaccination Card required if available.
Project H.O.P.E. is providing visits with medical staff to provide COVID-19 and Flu vaccinations to any and all persons. There is no cost to patients, but claims will be submitted to insurance plans and other funding sources to cover the cost of these visits. Charges for these visits range from $65.00 to $105.00 dependent on who sees the patient.
