POSITION SUMMARY
The Project H.O.P.E. Physician is responsible for providing quality primary care for Project H.O.P.E. patients. Responsibilities
1. Is a member of a Care Team and contributes to functionality of their assigned care team.
2. Works to implement team based care
3. Participates in Care Team meetings as scheduled
4. Function as an independent provider of primary health care to a multicultural, indigent patient population.
5. Insures the delivery of consistent and high quality professional service.
6. Contribute as a physician with skills in public health management and preventative medicine.
7. Meets best practice standards and sets the agenda for the medical practice.
8. Assist in the development of competency programs and customer services initiatives.
9. Responding to medical/health problems presented by patients including history taking, diagnosis, investigation, treatment and referral as appropriate;
10. Assess health status of patient through the collection of health data, physical assessment, interpretation of lab results and other diagnostic tools.
11. Prescribe and administer medications according to New Jersey State law.
12. Maintaining confidentiality and impartiality;
13. Coordinates and integrates the medical service with other patient care services to provide a continuum of care for the patient. Provides continuous care for patients and does not delegate responsibility for diagnosis or care of patients to a practitioner not qualified.
14. Participate in Quality Assurance and Quality Improvement Activities;
15. Complies with Medical Staff credentialing procedures, quality assurance and risk management initiatives.
16. Participate in clinical team meetings on a regular basis;
17. Commissioning health care by liaising with medical professionals in the community and hospitals;
18. Promoting health education in conjunction with other health professionals;
19. Organizing preventative care plans for individual patients;
20.Providing specialist clinics for specific conditions or for certain groups, e.g., diabetes, smoking cessation, new babies;
21. Managing disease states in accordance with national health guidelines
22. Complies with HIPPA regulations as they apply to private physician practices.
23. Discussing the development of new pharmaceutical products with pharmaceutical sales representatives;
24. Works collaboratively with Nurse Practitioners.
25. Arranges for coverage during vacations, and periods of absence from the office.
26. Is responsible for the quality control of medical care throughout the facility, including compliance with JCAHO, BPHC HRSA, and NJ Standards for Licensure of Ambulatory Care. Attends meetings for this reason.
27. Promptly notifies the Medical Director and Director of Project HOPE of any revocation, restriction, suspension, curtailment or surrender of professional license, membership or privileges at any hospital or other health care institution or the filing of charges by the NJ Department of Health and Senior Services or any U.S. law enforcement agency or health regulatory agency, or of a suite and/or claim against the practitioner alleging professional liability, or any formal proceeding before any third party provider or State agency, board, society pr any change in provider status.
28. Complies with organizational and departmental policies, procedures and standards of ethical practice.
29. Participates in HRSA OPR, HUD, JCAHO and NJ State inspections; insures compliance with prescribed standards.
30. Participates in the preparation process for HRSA OPR, HUD, JCAHO and other inspections including the collection of data.
31. Participates in programs, procedures and staff training.
32. Meeting daily and weekly productivity that has been set;
33. Using IT skills – This includes utilizing the electronic medical records by fully documented all patient care items in the electronic medical records, e-prescribing, as well as utilizing basic functions including Microsoft Office;
34. Keeping up to date with medical developments, new drugs, treatments and medications, including complementary medicine;
35. Instructs medical students rotating through Project HOPE.
36. Maintaining a portfolio of continuing professional development (CPD) activities.
37. Perform other related duties as assigned.
License, and Experience
1. Doctoral degree (MD, DO or Pediatrician) License to practice in the state of New Jersey.
2. Valid DEA and CDS licenses.
3. Preferred experience in an outpatient health clinic setting caring for undeserved populations.
4. Board certified.
5. Interest in participating in quality improvement programs.
6. Experience with managed care, administration of programs, and staff supervision, including staff at satellite clinics.
7. Preferred experience with Electronic Medical Records.
8. Valid driver’s license.
9. Excellent communication skills, collaborative interpersonal skills, and an outstanding record in clinical care and education.
10. Experience with working in Electronic Health Records (EHR) Knowledge, Skills, and
Abilities
1. Knowledge of federal, state, and other applicable standards for clinical practice.
2. Outstanding written and verbal communication skills.
3.Experience/Knowledge of JCAHO accreditation process and requirements, as well as all federal, state and local regulations and standards associated with the delivery of care in a community health center environment.
4. Ability to make patient related medical decisions on an hourly/daily basis.
5. Ability to provide leadership/guidance in a supervisory role and the flexibility to function as a team member at the management level.
6. Excellent managerial & supervisory skills with the ability to plan/organize and follow through in a timely manner.
7. Demonstrated ability to work with automated systems, including electronic medical records (we use EHS) and MS Office products such as Word, Excel and Outlook.
8. Ability to work collaboratively with diverse individuals and situations, including strong problem solving and conflict resolution skills.
9. Excellent innovation skills; requires development of new protocols, policies, and procedures for clinic function.
10. Proven understanding of quality improvement initiatives and collaboratives.
11. Experience dealing with underserved populations and cultural competency.
12. Ability to travel for training, collaboration with other organizations & agencies, oversight of satellite clinics, and to other areas of community as necessary.
